Not Every Case of Jaundice Is Hepatitis: Understanding the Causes - Dr Pradeepta Kumar Sethy

Jaundice, which shows up as a yellowing of the skin and eyes, is one of the more noticeable symptoms a patient can have, and it almost always raises concern about the liver. But there's a common misunderstanding that needs addressing: many people assume jaundice automatically means hepatitis. This isn't accurate. Jaundice is simply a symptom, not a diagnosis on its own, and there are several conditions beyond viral hepatitis that can cause it.

Understanding the Clinical Basis of Jaundice

At its core, jaundice results from a build-up of bilirubin, a yellow pigment released when red blood cells break down, in the bloodstream. In a healthy system, the liver takes on the task of metabolising this bilirubin and clearing it out via bile. Problems arise when that process is interrupted at any stage, be it through hepatic dysfunction, an obstruction somewhere in the biliary tract, or excessive breakdown of red blood cells. Once bilirubin begins to accumulate, it produces the yellow discolouration seen in the skin, the sclera, and occasionally the urine as well.

Hepatitis Represents Only One Category of Cause

Hepatitis, in strict clinical terms, refers to inflammation of the liver, and viral infections such as Hepatitis A, B, C, D, and E are its most frequent triggers. It is, without doubt, a significant contributor to jaundice, though far from the sole one. Clinically, jaundice tends to be grouped into three categories, depending on precisely where along the pathway the disruption occurs.

Pre-hepatic jaundice sets in before bilirubin has even reached the liver, generally as a consequence of red blood cells breaking down at a faster rate than usual. Malaria, sickle cell disease, and several inherited haemolytic disorders are typically responsible for cases falling under this category.

Hepatic jaundice, by contrast, stems from dysfunction within the liver itself. This is where hepatitis is classified, though it shares the space with several other conditions, cirrhosis, non-alcoholic fatty liver disease, hepatocellular carcinoma, and drug- or toxin-induced liver injury among them, none of which require a viral trigger.

Then there is post-hepatic, or obstructive, jaundice, which occurs when bile flow is physically blocked after leaving the liver. Gallstones, tumours, and structural abnormalities within the biliary tract are common causes here. Notably, hepatic function in such cases often remains unaffected altogether, with the actual problem sitting further down the biliary pathway.

Recommended Diagnostic Approach

Any instance of jaundice calls for prompt medical evaluation rather than assumption. The standard diagnostic workup usually includes liver function tests, serum bilirubin measurement, viral hepatitis markers, and imaging such as abdominal ultrasonography, all aimed at establishing the exact underlying cause. Additional symptoms, including fever, abdominal pain, dark-coloured urine, pale stools, or unexplained weight loss, can further help narrow down the differential diagnosis.

Don't Ignore the Warning Signs

Hepatitis undoubtedly remains one of the more recognised causes of jaundice, but medical professionals continue to stress that jaundice, at its core, is simply a clinical sign, an indication that something has gone wrong somewhere along the bilirubin metabolic pathway. The underlying cause could be something relatively minor and self-resolving, or it could point to a far more serious condition requiring urgent attention.

For this reason, patients presenting with jaundice are strongly discouraged from attempting self-diagnosis and are instead advised to seek prompt evaluation from a qualified physician. Identifying the precise underlying cause early not only guides the correct course of treatment but also helps prevent the complications that so often follow when a diagnosis is delayed or gets it wrong.
