Growing Role of Molecular Imaging in Prostate Cancer Care - Dr Ishita B. Sen

Prostate cancer is one of the most common cancers affecting men globally, and its burden continues to rise in India with increasing life expectancy and greater awareness leading to more diagnoses. While advancements in treatment have significantly improved patient outcomes, one factor continues to influence every clinical decision the ability to accurately determine the extent of the disease. As a nuclear medicine physician, I have witnessed how molecular imaging is transforming this aspect of prostate cancer care, enabling us to move beyond conventional diagnosis towards truly personalised management.

Traditionally, the diagnostic pathway for prostate cancer has relied on PSA (Prostate-Specific Antigen) testing, digital rectal examination, biopsy, and conventional imaging such as CT scans, MRI and bone scans. These investigations remain indispensable and continue to form the foundation of diagnosis and staging. However, they have certain limitations, particularly when it comes to detecting very small areas of disease or identifying recurrence at an early stage.

This is where molecular imaging has emerged as a game changer.

Unlike conventional imaging, which primarily identifies structural abnormalities, molecular imaging allows us to visualise the biological activity of cancer cells. Instead of simply asking where a tumour is located, we are now able to understand how the cancer behaves at a cellular level. This provides far greater confidence in staging the disease and selecting the most appropriate treatment strategy for each individual patient.

One of the most important developments in recent years has been the increasing use of Prostate-Specific Membrane Antigen (PSMA) PET-CT. PSMA is a protein that is highly expressed on prostate cancer cells, making it an excellent molecular target for imaging. By combining metabolic and anatomical information, PSMA PET-CT has significantly improved our ability to detect prostate cancer with greater precision than many conventional imaging techniques.

In everyday clinical practice, this has had a meaningful impact on patient care. For men with newly diagnosed prostate cancer, accurate staging is essential because treatment decisions vary considerably depending on whether the disease is confined to the prostate or has spread to lymph nodes, bones or distant organs. Better imaging enables us to tailor treatment more appropriately, helping clinicians decide whether surgery, radiation therapy, systemic treatment or a multimodality approach is likely to provide the greatest benefit.

Molecular imaging has also transformed the management of patients who experience biochemical recurrence after treatment. A rising PSA level following surgery or radiation therapy often creates uncertainty for both patients and clinicians, especially when conventional imaging appears normal. PSMA PET-CT can frequently identify recurrent disease at a much earlier stage, allowing timely intervention before the disease progresses further. Earlier detection translates into more informed clinical decisions and, in many cases, more effective treatment planning.

Perhaps the most exciting aspect of molecular imaging is its growing role in precision oncology. Modern cancer care is increasingly moving away from a one-size-fits-all approach and towards treatment strategies that are guided by the unique biological characteristics of each patient's disease. Imaging today is no longer simply about confirming the presence of cancer—it has become central to treatment selection, response assessment and long-term disease monitoring.

The scientific advances that have led to PSMA-based imaging are also supporting the development of targeted therapies for selected patients with advanced prostate cancer. Imaging and treatment are becoming increasingly interconnected, allowing clinicians to identify patients who may benefit from newer therapeutic approaches based on the molecular profile of their disease. This integration of diagnostics and therapy represents one of the most significant developments in contemporary prostate cancer care.

At the same time, it is important to recognise that advanced imaging is not necessary for every patient. The decision to perform molecular imaging should always be individualised and based on clinical findings, disease characteristics and current evidence-based guidelines. Optimal patient care is achieved through multidisciplinary collaboration involving urologists, medical oncologists, radiation oncologists and nuclear medicine specialists, ensuring that every treatment decision is both appropriate and patient-centric.

While technological innovation continues to reshape prostate cancer management, one principle remains unchanged—early evaluation remains the cornerstone of better outcomes. Men should not ignore persistent urinary symptoms, blood in the urine, unexplained bone pain or rising PSA levels. Timely consultation with a healthcare professional allows appropriate evaluation, accurate diagnosis and access to the most suitable treatment options.

The future of prostate cancer care lies in precision medicine, and molecular imaging is playing an increasingly important role in making that vision a reality. By improving diagnostic accuracy, enabling earlier detection of recurrence and supporting personalised treatment planning, these advances are helping clinicians deliver more informed, targeted and effective care. Ultimately, our goal is not merely to detect prostate cancer more accurately, but to ensure that every patient receives the right treatment, at the right time, based on the unique biology of their disease.
